Boba's Helmet - The aerial thing on Boba Fett's helmet switches sides.
Han's Shirt - When Han is released from Carbonite, he falls forward, dirtying up his shirt. In the next shot, his shirt is clean.
Skiff Fall - Han falls off the skiff feet first in one shot, then head first in the next.
Luke's Kick - When Boba flies into the air you can see Luke kick at someone and obviously miss, but the person flies back anyway.
Lando's Rank - Lando's rank symbols switch sides during the meeting with Han before Han leaves.
Jabba's Stripper - When Oola and Jabba are fighting over the chain, Oola pulls back hard enough for her costume to slip and show her breast. This is fixed in the Special Edition.
Lando's Headache - When Chewie is being taken to the dungeon, Lando smacks his head hard enough to knock his helmet partly off. Billy Dee Williams (actor of Lando) was taken to a nearby hospital with a concussion.
Skiff Jump - When Luke jumps off the skiff, he his seen grabbing the plank with one hand, but in the next shot is seen grabbing with two hands.
Leia's Helmet - Leia takes off her helmet twice while talking with Wicket (the first Ewok).
Lando's Holster - Lando's shoulder holster switches sides a couple of times while having the conversation with Han in the hangar.
TIE Fighter - At one point in the Battle of Endor, a TIE Fighter magically appears in the middle of the Milennium Falcon.
Radar Dish - The Falcon's radar dish is shown getting knocked off while flying inside the Death Star. In later scenes it is shown with it on still.
